All children, youth and adults who work with children or youth must have a release form on file.

Children’s Release Forms

All children (birth – 5th grade) participating in St. Paul’s activities must have a release form on file each year. Download and fill out the Youth Release Form and return it to Amy Heitman.

Youth Release Forms

All youth (6th – 12th grade) participating in St. Paul’s activities must have a release form on file each year. Download and fill out the Youth Release Form and return it to Jodi Stuermer. Those attending youth group must also fill out the Youth Group Off Campus RELEASE AND WAIVER OF LIABILITY

Adult Background Check Release Forms

All adults who work with children or youth must have a background check performed each year. Download and fill out the Release form for Adult Background Check and return it to the church office.
